Assessment Completion Certificate

After your assessment is finalized, you may view and print a certificate of completion.  Be sure your browser's popup blocked allows popups from CMS Online.

A certificate icon will appear in two places after your assessment is final.  Select My Reports from the left navigation menu to access Assessment Results or Assessment History reports.

Clicking the certificate icon will open another window where your certificate will display.  The QR code may be scanned to access the certificate online.
